Please advise reception as soon as possible if you change your name (eg upon marriage), address and/or telephone number.
In the event of a change in name, we ask that you provide the relevant deed poll or similar verification as evidence which we will duplicate for our records.
In the event of a change of address, we ask that you provide recent evidence of residency, e.g utility bill, council tax form which again we will duplicate for our records.
We currently will only accept new patients that fall within our practice boundary. Please contact us to find out if you fall within our boundary line, or alternatively contact the Local Health Board who will provide you with an up to date list of GP surgeries within your area.
In accordance with our policy for patients moving to an address outside our practice boundary, it is necessary to register with a doctor closer to your new address.
To re-register simply call in to a doctor of your choice with your medical card or alternatively they will provide you with a form to complete.
If any other members of the houshold currently registered have also moved, they too will need to register with a new doctor.
